On 28 December 1919 a bronze tablet was dedicated to the memory of 265 soldiers and nurses of the congregation who served in the First World War by Reverend James Fielding Sweeny, Bishop of Toronto, and unveiled by Commander of the Canadian Corps General Sir Arthur Currie. The Church of the Epiphany was located at 1515 Queen Street West until it was sold on 11 June 1983 to the Our Lady of Lebanon Parish.

The Window and Oak Panelling in the Chancel and this Tablet have been erected by the Congregation to perpetuate
the memory of 265 Soldiers and Nurses from the Church of the Epiphany: whose names are given above and who enlisted for service in
defence of the Empire in the Great War of 1914-1919: forty-one of whom laid down their lives in the cause of Freedom, Righteousness and Humanity.
