This plaque honours 21 men from the Elm Street Methodist Church congregation who laid down their lives in the Great War. It is not known when the plaque was unveiled. It was transferred to the Metropolitan Methodist Church when the two congregations amalgamated in 1922 and was dedicated on Sunday, September 9, 1923, in its new location. In the same service, a memorial window and plaque were dedicated to honour those from the Metropolitan Methodist Church congregation who died during the Great War.

Elm Street Methodist Church was founded in 1853. It closed in 1922 and the congregation amalgamated with Metropolitan Methodist Church.
