Overview:

The Fredericton Homeless Shelters Inc. (The Shelters), with funding from the Veteran and Family Well-Being Fund, operated a program called The Continuum of Care. The program aimed to provide permanent housing to homeless Veterans in Fredericton, New Brunswick and surrounding and connect them with the programs, services and supports they need to find stability in their lives both during their time at The Shelters and post-shelter residency.

Project goals:

The project’s goals were as follows:

  1. Provide as many homeless Veterans as possible in the Fredericton, NB area with permanent housing,
  2. Connect homeless Veterans with the programs, services and supports they need to find stability in their lives, and
  3. Continue to support Veterans after their residency at the Shelters.

Project activities:

The project’s activities included the following:

  • Complete Veteran shelter intake and collect intake data,
  • Connect Veterans with Integrated Service Delivery workers,
  • Complete assessments and connect Veterans with Veterans Affairs Canada, and
  • Assist with housing set-up and provide housing supports post-shelter.

Project results:

The program provided shelter, activity and housing support to 12 Veterans (11 men and 1 woman) and found permanent housing for four of the 12. As well, they provided ongoing support to at least three Veterans at any given time.

Overall, they strengthened their relationships with other agencies to enhance service coordination and delivery over the long term.
