Service health records for disability benefit application

A disability benefit is a tax-free payment to support a Veteran’s well-being. To qualify for a disability benefit you:

  • must have a diagnosed medical condition or disability; and
  • show that the condition or disability is related to your service.

We will obtain your health records

We will get a copy of your service health records when we receive your application for disability benefits.

Under the Pension Act and the Veterans Well-being Act, we have the authority to receive your records directly from:

Only records received directly from the Department of National Defence or Library and Archives Canada will be used for decision making purposes. Your records will be reviewed as part of your application.

You do not need to personally make a request for your records. We will not be able to receive them directly from you.

You may request your service health records

You have the right to request a copy of your personal information held by a government institution through the Privacy Act. You may also request corrections or have a notation added to any recorded personal information. Request your records from:
